To receive an "S" Grade on your transcript the following requirements must be completed.  To assist you in the process a timeline has been provided.

1. Employment Information Sheet:
   Log onto your ISU CMS account (https://ecms.eng.iastate.edu/students/) to complete the Employment Information form. After logging into your account, click on 'Profile' in your main header. Next, click on the 'Placement' tab. You will then choose the 'Edit Evaluation' button for your current work assignment. Please complete the sections titled 'Student Employment Information, Supervisor Information and Mentor Information (Mentor Information is optional).  After completing these sections, scroll down and click on the 'submit' button.

3. Student Evaluation of Work Form:
   Log onto your ISU CMS account  (https://ecms.eng.iastate.edu/students/) to complete this form.  After logging into your account, click on 'Profile' in your main header.  Next, click on the 'Placement' tab.  You will then choose the 'Edit Evaluation' button for your current work assignment.  Please complete the Student Evaluation of the Experience and Evaluation of Work Experience and Supervision sections.
